SFD Provider Access
Existing Providers in Service Fee Directory
- Click on “Provider – User Login” button and on the Login page click on Reset/Forgot password below the Login button.
- Scroll the dropdown list and select Service Fee Directory from the options in the list and enter your Registered Email Address Search
- Click on Submit button. Follow instructions on the screen and an email with instructions shall be sent to the registered email address.
